▶Objectives
The project is to improve the quality of life and social inclusion of people with Alzheimer or other forms of dementia at risk of social marginalization, as well as of their families and carers. The proposal contributes to giving effect to those rights in the everyday lives of people with dementia and their families, improving the services and support available in the communities of intervention, with a focus on intergenerational connections between people with dementia.
▶Activities
Partners will organize and take part in 3 exchange visits that will be realized in the respective countries, focusing on community integration and intergenerational connections practices implemented by the peer organizations. Partners will develop a best practice toolkit including a training module and a pilot. It will organize a multi-channel dissemination campaign and partners will present the results of the project in local and international events.

Created a set of tools aimed to improve services for a better quality of life and inclusion of people with dementia and increase competences; implemented 1 pilot project on intergenerational and community inclusion; People living with dementia and their families have been informed about the enhanced support services available and the dissemination campaign has raised awareness across dementia with key stakeholders to change behavior, promote engagement and access to services.
