Objectives

HealthEU aims to strengthen the knowledge, skills, and abilities of social workers and community organizations, create the educational HealthEU Toolkit for social workers, raise awareness about diabetes prevention and management through a focused communication campaign, promote the active involvement of citizens in sports and physical activities to prevent diabetes, and widely share project outcomes to ensure maximum impact.

Activities

The project will implement a series of activities, starting with research to collect data on diabetes and inclusion. Focus groups with local stakeholders will gather insights to inform the development of the HealthEU Toolkit, a comprehensive resource for diabetes awareness and care. Capacity-building sessions will equip social workers and healthcare professionals with practical skills. Additionally, the project will organize local events to raise public awareness and a final event.

Impact

The project will strengthen the capacities of partner organizations and social workers in diabetes prevention, care and social inclusion, increase awareness among target groups about diabetes management, develop the HealthEU Toolkit as a valuable resource, create a stable partnership network for exchanging good practices, and ensure effective dissemination and use of project results for long-term sustainable impact through focus groups, capacity building, and public events.
